Registration fees for congress participants included:

Registration only became effective once the registration form and payment was received by the Congress Secretariat. Every participant who had completed the registration process received a registration number and a confirmation of registration.Â
Early fee: until and including 15 May 2011
Normal fee: from 16 May to 25 July 2011
Late fee: from 26 July to 24 August 2011

SICOT is legally obliged to charge 20% Czech VAT on all fees quoted. According to the European Union VAT Directive, VAT of the country hosting the congress must be charged. Article 53 of the VAT Directive states that "cultural, artistic, sporting, scientific, educational, entertainment and similar activities are taxed at the place where those services are physically carried out".
IMPORTANT NOTE: The presenting author must have registered and paid the congress registration fee before 15 June 2011 to have his/her abstract included in the Final Programme. The Congress SecretariatÂ did not check if co-authors had registered. Abstract submitters can change the presenting author of an abstract through the abstract submission system until 15 June 2011, by clicking on the link in the confirmation email received after submitting the abstract. Please ensure that the family name, given name, institute, and address of the presenting author have been inserted in the system.

Please note that participants who registered as Active Members or Associate Members will only receive their confirmation of registration and invoice onceÂ their 2011 membership dues were paid.
Participants who registered as non-member trainees only received the confirmation of registration if they had supplied evidence that they are under 40 years of age and actively engaged in a training programme. Please provide the Congress Secretariat with a copy of your passport or birth certificate and a letter from your supervisor.Â Â Â
To pre-register groups of over 10 participants, please contact the Congress Secretariat.
Under EU law, companies which are registering participants and are located within the European Union must supply their VAT (Value Added Tax) number.
The Opening Ceremony & Welcome Reception, Presidents' Dinner and Fun Night were NOT included in the congress registration fee.Â More information about the Social ProgrammeÂ is available here.Â
The Charity Run and Golf Tournament were NOT included in the congress registration fee.Â More information about the Sports Programme can be found here.

If you need an invitation letter for visa purposes, please fill in the relevant field on the registration form. If you have already registered and need an invitation letter, please contact the Congress Secretariat at email@example.com, mentioningÂ your passport number in the message.
Participants who cancelled their registration before 31 May 2011 will receive a 75% refund of the amount paid.
No refunds will be made for cancellations received after 31 May 2011.
Name changes incurred a charge of EUR 30 per replacement.
A handling fee of EUR 35Â was retained in the event of cancellation of social activitiesÂ before 31 July 2011. No refunds will be made for cancellations of social events received after 31 July 2011.Â
Refunds will be processed after the Congress.Â Â
